A frenum is a band of tissue in and around the soft tissues of the mouth, more specifically the gums and teeth. There are different types of frenum: lingual, labial or buccal.
Lingual frenum - a band of tissue between the tongue base and the floor of the mouth. When it is hyperactive or excessively limiting , in this region it is called Ankyloglossia. This commonly is referred to as being "tongue-tied". An ankylotomy procedure is required to allow for improved eating, speaking or self-cleansing.
A Labial frenum is a thin band of tissue which typically attaches the movable tissue of the cheek/lip (non-keratinized tissue) to the attached tissue (keratinized) around teeth. When located in this region it can lead to periodontal problems and /or orthodontic issues. It can restrict the teeth from coming together appropriately. In the case of a patient who wears upper/lower prosthesis', the frenum can limit the ability of the patient to tolerate normal prosthetic function and thus, is commonly relieved.
When the frenum is positioned inappropriately/excessively it can limit tongue movement in the case of a lingual frenum or it may lead to soft tissue recession in the case of a labial frenum.
The frenectomy is a simple surgical procedure. The attachment is conservatively relieved to address the specifics of its situation and return the patient to more normal, less restrictive function. It is well tolerated.
